Understand the score before you begin
Rate each statement based on how consistently it is practiced in your current operation—not what your procedure says should happen. Answer with your typical experience during the last 90 days.
Choose “Not sure / no evidence” when the practice cannot be confirmed. The report will identify these areas for follow-up.
Your percentage is a MAYOWA OPEX self-reported practice maturity score, not your actual warehouse efficiency, audit grade, or industry percentile. We show external KPI comparisons separately when you enter actual measures.

How your percentage is calculated
Each of the 24 questions can contribute up to four points. The overall percentage is total points divided by 96, multiplied by 100. Each operating area contains six questions, worth up to 24 points. “Not sure / no evidence” responses are identified for follow-up and are not counted as demonstrated practices.
Your four operating areas
What your grade means
85–100% · Strong foundationMost reported practices are consistently established. Validate against measured performance and exceptions.
70–84% · DevelopingCore routines exist, but inconsistent practices or evidence gaps deserve attention.
50–69% · Improvement prioritySeveral controls or routines appear unreliable and warrant structured investigation.
0–49% · Foundational attentionStart with critical workflows, ownership, and basic measurements before broad changes.
These four grade bands are defined by MAYOWA OPEX for education. They are not independently validated industry standards or a measure of how you rank against other facilities.
Your operating profile
What to investigate next
These are investigation hypotheses based on your answers, not confirmed root causes. Start with your lowest-scoring practices, collect real evidence, and verify before implementing changes.
Recommended action plan
Validate the two lowest category scores with shift observations and documented data.
Select one recurring problem and define its baseline: frequency, location, impacted work, and owner.
Test a limited corrective action and record what changes.
Reassess after 30–60 days to determine whether improvements are sustained.
Industry metric reference points
Professional warehouse frameworks emphasize metrics such as inventory count accuracy, dock-to-stock cycle time, on-time shipment, order picking accuracy, and capacity used. Benchmark comparisons require consistent metric definitions and similar operating conditions.
Published reference points
APQC: Median inventory accuracy 95%; median dock-to-stock cycle time 15 hours. The published distribution includes different company and operation types and is not a universal target. Sources: APQC Inventory Accuracy and APQC Dock-to-Stock.
WERC: Its DC Measures program tracks inbound, outbound, capacity, quality, workforce, and customer metrics; benchmarks should account for facility type and work mix. Source: WERC DC Measures.
Compare your real KPIs (optional)
These fields never change your self-assessment score. Use the same calculation definitions and reporting period as the source before comparing.
A better or worse figure than a published median does not establish operational health on its own. Use work mix, system definitions, sample size, and process observation to interpret differences.
